Release of Contact Information form
At CARNA's discretion, a researcher may have direct access to the
contact information of individuals willing to participate in research.
A Release of Contact Information contract must be completed and
returned to CARNA prior to release of the information. This release
details the collection, use and disposition of the personal
information provided by CARNA that the researcher must comply with.
CARNA does not conduct email surveys on behalf of researchers.
